Use this form if you are being requested by an outside organization to supply a copy of an insurance certificate with them named as additional insured or to provide proof of coverage. Club Waiver – 18 Years of Age and older

This waiver should be signed by anyone who has not submitted an online waiver to the OCA or who is participating in a try-out activity with the club. Note clubs can request that this waiver be physically signed and handed in, in addition to the online waiver through CCN Bikes. Note: Youth Members under 18 years of age should use the Under 18 Youth waiver as it requires parental consent. Please fill out this form if you are requesting to upgrade your ability based race category. Riders may not change their category from Elite to Master or vice versa during the season. NOTE: This form is not required to be completed if the athlete has the required points to upgrade but they must still contact the office to arrange this change on their licence. Please see various upgrade lists posted in the results section to confirm. Upgrade Requests are reviewed periodically, not daily. Insurance: 2020 Commercial Event Application

Any level of event that intends to allow member and/or non-member participation is required to submit a request for commercial event insurance. The organizer will pay the premium for the event prior to event day and reconcile insurance costs based on participation numbers following the event.
